Mary Jennings and Associates Business Health Check Questionnaire

Three sets of questions to assess the health of your business follow. Rank each question by selecting the number between one and ten that most reflects your position.

Example: Business Planning

The business plan for my firm is fully communicated to all staff members.

1 = Least accurately reflects my position
2
3
4
5 = neutral - neither high nor low
6
7
8
9
10 = Most accurately reflects my position

Each score will be used to calculate an index that will reflect the present status of your business. A score of eight will be shown by placing a cross in the box beside “8” on the scoring sheet/score pad.
